In 2007 The Royal Children's Hospital, Melbourne (RCH) had a problem. Outpatient letters, dictated by clinicians following clinical consultations, were regularly typed at least four weeks after the patients had been treated. To make matters worse, the process by which the physician approved the contents once the letter had been typed was inefficient, further adding to turnaround times. This process involved sending hard copies of the letter to the clinics, then waiting for the doctor to return to the clinic (not necessarily the next day or even the next week), at which time the doctor would make any necessary corrections on the hard copy. The corrected version would then be sent back to Medical Transcription Services (MTS), the changes made, then the letter printed and mailed out. The process could be described as being convoluted and time consuming, with double handling presenting opportunities for delay.
